Sports Physical
*IHSAA Guideline sports physical.
Wellness Visit
*Complete physical exam, including orders for lab work or other necessary testing.
UTI Visit
*Screening for UTI with treatment.
Hormone Services
*Initial and follow-up visit: full physical exam & discussion of results.
*Monthly TRT Treatment.
*Pellet therapy.
STI Visit
*Sexually transmitted infection/illness screening appointment.
